Students working to complete a capstone or thesis project beyond the time they are taking classes or those needing library privileges for a Continuing Education class should fill out this form. A photo I.D. (driver’s license, passport, etc.) is needed to obtain library privileges. Allow one full week for library privileges to be activated.
