Brief focused summary of experience: (2-3 sentences)

Elective Credit: Please indicate topic content of an equivalent course.
Pre-approval is required for an essay to apply towards general education credits.

This is a re-submitted essay. Essay rewrites will be accepted up to 90 days from original submission date.

I have read and understand the following agreement statements. Before essay will submit, all boxes must be checked including signature and date.

Only one rewrite will be allowed when credit awarded is zero
Credit awarded will range between 0-4 credits per essay.
A nonrefundable essay fee of $125.00 must accompany each essay submitted.
Essay should contain a minimum of three(3) properly cited outside sources.
Essay must have an introduction and conclusion.
Essay must include all stages of the Kolb model.
Credit will not be awarded if student has previously received credits for this topic. If you have already been awarded more than 32 non-traditional credits please contact your academic advisor.
